for the 2008 event. You
will be contacted promptly after the submission of this form.
Once you review and complete the Local Site Agreement and fax
it in (or if you have completed this process
online),
your club, subject to acceptance by us, will be added to our
2008 Host Sites page with a link back to your web site.
You may download the necessary documents and information from
this page in the Adobe (PDF) format. You
can return to this page at any time by Clicking on the
photo of the pro postcard on our Homepage and then Clicking on "2008"
at the top of the Pro Event information page.

Regional Finals Site Request
If you would like to be considered to host a Regional
Finals Tournament in your area, please download and complete
the information contained in this package. |

| If you attending the National Finals, collecting a check, or are selected as a Regional Finals Host Site you
will need to complete a W-9 form (separate one required for Regional Finals Site) to allow us to set you or your
facility up for payment. We will direct you here to download
this form or fax the document to you when necessary. |

For 2008 the suggested entry fee will remain at $60 per player with
sites retaining $15 per entry fee commencing with the first player.
Therefore sites will remit $45 per player to ESPN from each fee, plus applicable Regional Finals fees collected from the players.
We think the consistency of this revenue sharing will help simplify
the settlement process.
The Callaway HX Tour Balls will be delivered
to the Local Host Site prior to their event for immediate fulfillment
to the participants.

National Finals will be broadcast on ESPN 2. Gross and Net 2-person teams from all
over the U.S. will compete to determine who will be 2008's "America's
Best 2 Some".
